This service involves a consultation with family and loved ones where we will gather information 
and provide education, referral and advice in reference to your needs.
We will provide you with information and advice relating to your care requirements including local support services, as well as information relating to other community and government funded resources available to you. If required we will assist you in:
- Completing applications and referrals for services
- Assist in setting up a care program
- Access to the clinical nurse consultant for education and support in providing care to loved ones
- Assist with future planning
