Nursing Care – Ongoing Support

nursing-care-outgoing-support-services-nurses-dementiajpgOur unique program offers a combination of Case Management, clinical nurse consultancy specializing in Dementia Care and access to a core group of nursing staff with extensive Dementia training and care experience.

Over the course of their Dementia, your loved one will require increased assistance to maintain dignity and independence.  Our Consultancy offers a core group of Dementia specific nurses that can be employed privately, by you, to support your loved one.

The level of care required will be discussed with you at the initial consultation and can be provided flexibly to meet your loved ones care needs, budget and allow you the freedom to live your own life. All care provided through our consultancy includes access to a Clinical Nurse Consultant who will oversee the nursing component of the program including liaison with medical professionals (i.e. GP, Geriatrician and Psychiatrist).

Care can be provided as:

  •  Full 24 hour care programs
  • Individualized blocks of care
    (i.e. to assist with showering, meals, community access
    and taking to appointments)
  •  Overnight care
  •  Crisis care
  •  Planned respite care – to relieve families from the everyday pressures
    of caring for a loved one with Dementia.

The services provided by our nurses include:

  •  Ongoing Assessment as to changing care needs
  •  Behaviour Management
  •  Medication Management
  •  Wound Care
  •  Continence Management
  •  Documentation and Charting
  •  Managing Diet and Nutrition, including preparing meals
  •  Personal Care - showering, dressing and grooming
  •  Support with shopping, cleaning and errands
  •  Support with Socializing / Companionship and Leisure Activities
  • Attend Appointments
